New Patient Registration

The doctors welcome new patients who live within our practice area. 

The practice covers the whole of Pitsea and Basildon. The surgery is situated in the East Wing of the Dipple Medical Centre.

How to Register

Patients wishing to register will need to provide their NHS medical card or complete a family doctor services registration form and provide 1 form of photo identification and 1 proof of address (utility bill). We will also ask you to complete a new patient questionnaire which provides a medical history until we receive your records from your previous surgery. If you are on regular medicine please bring evidence of this with you. 

Once your application is complete an appointment will be made for you to attend for a health check with our practice nurse.

You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

All new patient registrations please come in between 2pm and 5pm to register.

New Baby Registrations

Where possible we would ask you to register the new baby prior to six week postnatal check so we can ensure baby’s registration is complete before their first immunisations are due.
